Step 3: Workflow Launch & Care Plan Generation If the patient is eligible:

  • Lyrebird launches a pre-filled, review-ready care plan workflow

  • The GP sees a simplified CCM interface with the correct forms, fields, and billing guidance β€” all pre-configured

  • No need for additional templates or manual admin

  • A new care plan can be generated or an existing one reviewed and updated with ease

Step 4: Action and Billing Once the care plan is reviewed or created:

  • Lyrebird updates the clinical record automatically

  • It ensures correct billing using MBS item 965 or 967

  • Required clinical notes are captured to support Medicare compliance
