Open Lyrebird through the patient file in Best Practice.
Obtain patient consent for recording and documentation.
On the right-hand side, you'll see the available workflows (965, 967, and 2715). For an initial appointment, select Item 965 and click Start.
Review the talking points or plan outline that appears. These talking points are provided because the 965 template has specific required fields that must be captured during the consultation.
Structure your consultation according to the talking points to ensure all fields will be filled out in their entirety. The key discussion areas include:
• Each chronic condition the patient has
• Management plan for each specific chronic condition
• Requirements for multidisciplinary referrals
Conduct your consultation. Lyrebird will capture all discussions and automatically integrate patient data from Best Practice, including:
• Current medications
• Known allergies
• Medical history
• Family history
When finished, click Stop Listening. Lyrebird will generate a pre-filled document using all the discussions and Best Practice data.
Review the generated care plan. You'll see:
• Patient details
• Medical History
• Current Medications
• The discussion during the consultation
If you discussed multidisciplinary referrals (e.g., physiotherapist, psychologist), you can generate referral letters directly within the care plan. Click the option to generate the letter for the specific provider.
Review each generated referral letter. If it looks good, send it directly from the system. Then return to the care plan to continue your review.
Once you're satisfied with the entire care plan, click Save to save it directly to Best Practice. The document will be saved in the "Correspondence Out" section.
Chronic Condition Management Plan
Lyrebird integrates with Best Practice to streamline the creation of Chronic Condition Management Plans. The system uses patient medical history from Best Practice and your consultation discussion to automatically populate all required fields.
